        My child has permission to engage in all physical education activities.

        During the school year, information may be released in various publications (yearbook, athletic programs, news articles, newsletters, etc,) about student activities. I hereby give my permission for Steeleville CUSD #138 to release pertinent information about my child.

        I hereby give my permission for my child to have his/her picture published on the school's website (names and pictures will not be posted together).

        I have received a copy of the Authorization for Internet Access and will discuss it with my child. I hereby give my child permission to use the Internet based on the school policy. I recognize my child may be subject to discipline for any inappropriate use of the District's network/Internet connection.

        I acknowledge receiving and/or being provided electronic access to the Student/Parent Handbook on student behavior. I understand that the Student/Parent Handbook may be amended during the year and that such changes are available on the School District website or in the school office. I recognize that my child is responsible for being aware of the Handbook rules and regulations and may be subject to discipline for failure to abide by these rules.

        My child may attend all field trips throughout the school year. I understand the classroom teacher will provide specific information prior to any field trip.

        I give my child permission to view curriculum and school appropriate movies/videos. Throughout the school year, some classes watch films/documentaries/movies/video clips to help convey meaning of the content being taught. Some of these films are unrated or rated PG13/R.

        I have received a copy of the Steeleville H.S. parking regulations and will discuss it with my child. I recognize that my child is responsible for being aware of the parking regulations and may be subject to discipline for failure to abide by these rules.

        I would like to be included in the ONE CALL NOW messaging system.

        Two federal laws, the No Child Left Behind Act of 2001 and the National Defense Authorization Act for Fiscal year 2002, require high schools to provide military recruiters, upon request, access to secondary school students and directory information on those students. If you prefer that your child's directory information (names, addresses, & phone numbers) not be released please check the "no" box.

This information will help identify military families in order to ensure that they receive federal and state aid to help prevent deployments disrupting the lives of our children and schools. Your participation will help your school get free help for children struggling with their parent/guardian's deployment, may help your school receive additional funding, and help your school use military education liaisons more effectively.
